The Fine Print
Commitment? Yes. 3 months for all memberships, 6 months for ANY add on that includes a service like DEXA scans, labs or medications. After these respective periods you can cancel with a 30 day notice via text, email or portal message.
What is "a visit" for members?: Anything requiring medical decision making. We maintain very open communication with our members and encourage general questions. Any explanation of new or worsening symptoms, questions about needing to go to the ER etc. are generally considered a visit. We try to be very accommodating for our members and avoid extra bills when reasonable.
Discounts? Yes. The memberships include discounts on the vast majority of extra services that we offer.
Gifting our services at the discounted rate? Yes, just don't abuse it.
Unlimited messages? Yes. If something requires an appointment, we will let you know, but we generally only require appointments for our members when necessary.
After hours responses? Yes. We keep our patient volume small on purpose. Messages are checked periodically on nights, weekends an holidays. If something seems time sensitive, or we have the time at that moment - we will respond. All patients as an "SOS" option can call the Performance Health main line and be forwarded to the provider's personal cell phone.
What are "necessary" labs?
CBC (complete blood count): up to 4 per year
CMP (comprehensive metabolic panel): up to 4 per year
TSH with reflex to T3 and T4: up to 3 per year
Progesterone level: up to 1 per year (women only)
Estradiol level: up to 3 per year
Lipid panel: up to 2 per year
Testosterone, total and free: up to 3 per year
Sex hormone binding globulin (SHBG): up to 2 per year
Ferritin level: up to 3 per year
LH and FSH: up to 2 per year
Hemoglobin A1c: up to 2 per year
PSA: up to 2 per year (men only)
Vitamin B12 and folate: up to 1 per year
Vitamin D : up to 2 per year
Unused tests do not roll over or accumulate.
