Transitional Primary Care for Those in Need
We offer transitional primary care services for patients who fall into any of the below categories:

Were seen in an acute care setting such as a hospital, ER or rehabilitation center.

Are uninsured.

Are uninsured and established with primary care but cannot get in to see their primary care provider soon enough for an acute issue.

Are otherwise in need of a reduced fee.

All are at the discretion of the provider and the practice.
Our Transitional Care Services Include

Reduced rate office visit of $99 per visit, for a maximum of 3 visits in a 24 month period.

Visit notes faxed to your primary care provider.

We will waive prior authorization fees when medication is deemed necessary. ​

Additional Requirements and Caveats

Medicaid (ACCCHS) does not cover labs ordered by our practice.

We CANNOT see medicare patients in ANY capacity - even if willing to pay cash.

Visit fee is due at time of service and does not include tests, labs, medications or procedures.

Patients must retrieve an upload their own medical records.
-
Patients must communicate via the EHR portal (no Spruce or text).
-
Visits must be during standard hours: M-F 8:30a to 2:30p.
