Booking Requirements:

New Patients

A $20 deposit is required to book an appointment. New patient mobile visits require a $100 deposit.

If payment is not made within 2 hours of booking, your appointment will be cancelled. ​​

Forms must be filled out within 24 hours of booking. If your appointment is within 24 hours the forms should be filled out immediately.

Card on file is subject to cancellation policy below.

Established Patients

No deposit required, card on file subject to cancellation policy below.

​Cancellation Fees:
-
Appointments canceled with less than 12 hours’ notice and "no shows" will be charged the full visit fee, including any applicable convenience, procedure or mobile service fees for a maximum fee of $500.

Appointments cancelled with more than 12 hours notice but less than 24 hours notice will be charged 50% of the total visit fee for a maximum fee of $250.

Appointments cancelled with more than 24 hours' notice but less than 48 hours notice will be charged 25% of the total visit fee for a maximum fee of $125

Appointments cancelled with more than 48 hours’ notice will not be charged a fee.​

For free consultations the "visit fee" used to determine the cost of no shows/cancellations is $200.
Time:

We find it very important to stay on time for our patients and spend adequate time with our patients.

We will accept late patients whenever possible as the discretion of Performance Health and our providers. There is a grace period of 8 minutes. Patients who are over 8 minutes late will be charged a $25 fee if we are still able to accommodate them.

If the provider cannot accommodate the late patient it is considered a no-show. ​

For free consultations $20 deposit will not be returned for lateness beyond the grace period.

​*ALL fees may be waived or reduced at the discretion of Performance Health in the event of a legitimate emergency or hardship and patient's working relationship and appointment history.
